Consent to the processing of personal data
In accordance with the requirements of Article 9 of Federal Law No. 152-FZ dated 27.07.2006 "On Personal Data", I make a decision to provide my personal data and freely, voluntarily and in my own interest, I consent to the Joint-stock company Agency for Development and Investment of the Omsk Region, OGRN 1115543006204, located at 644074, Omsk, ul. 70 let Oktyabrya, 25 building 2, office 506 (hereinafter referred to as the Agency, Operator), for processing (collection, systematization, accumulation, storage, clarification (updating, modification), extraction, use, transfer (distribution, provision, access), depersonalization, blocking, deletion, destruction) of my personal data, including: last name, first name, patronymic, gender, date of birth, place of residence, contact phone numbers, as well as other information related to me as a personal data subject, in order to receive information and consulting, as well as other Agency services.
In the process of providing services by the Operator, I consent to the dissemination and transfer of my personal data to other officials of the Operator in the interests of the Operator. I grant the Operator the right to perform all actions (operations) with my personal data within the framework of the Operator's interests that do not contradict the legislation of the Russian Federation. The operator has the right to process my personal data both by entering them into an electronic database, and in the mode of non-automated processing, with the direct participation of a person, inclusion in lists (registers) and reporting forms.
The Operator has the right, in fulfillment of its obligations to work with government agencies, to exchange (receive and transmit) my personal data using tangible media and through communication channels in compliance with measures to protect them from unauthorized access, provided that their reception and processing will be carried out by a person who is obliged to keep confidential information.
The transfer of my personal data to other persons or their other disclosure may be carried out only with my written consent, except in cases where this is necessary to prevent a threat to my life and health, as well as in cases established by the legislation of the Russian Federation.
This consent is valid for an indefinite period.
This consent may be revoked by the personal data subject. Upon receipt of the withdrawal, the Operator is obliged to stop processing them within 14 calendar days from the date of receipt by the Operator of a written document on the withdrawal of this consent.
I reserve the right to withdraw my consent by drawing up an appropriate written document, which can be sent by me to the Operator by registered mail with a delivery notification, or delivered personally against receipt to the Operator's representative.
